An MRI Experience: The Frustration of Unnecessary, Confusing and Incomplete Instructions!

Magnetic resonance imaging (MRI) is a test that uses a magnetic field and pulses of radio wave energy to make pictures of organs and structures inside the body. I needed this test to determine the extent of my uterine cancer. My recent learning experience with having an MRI reinforced the importance of giving meaningful, clear and complete instructions.

1. Unnecessary Instructions

My appointment instructions explicitly told me to wear pants without any metal attachments. Okay, that was no problem. All I had to do was wear pants with an elastic waist instead of a zipper. Unfortunately, I overlooked the second requirement until I had my snow boots, winter coat, scarf, hat and gloves on. Apparently, I was also supposed to wear pants without pockets.

This created some stress on my part. Who owns pants without pockets? I certainly don't! Actually, what is the point of pants that have no pockets?

Once we got to the hospital, there was no problem. They handed me pajama bottoms to wear. I guess I was one in a long line of patients who were not pocketless pant owners!

This begs the question- why give those pants instructions at all? A good rule of thumb for hospitals, trainers and life itself is to avoid giving unnecessary instructions!

2. Confusing Instructions

The instructions on the bottle of prescription sedatives indicated that two sedatives should be taken 30 minutes before the MRI and another two sedatives should be taken 30 minutes after the MRI.

Taking them beforehand made perfect sense. I am claustrophobic and the idea of lying completely enclosed for an hour gave me great concern, to say the least. The sedatives were intended to take the edge off and enable me to relax and stay still.

However, I couldn't imagine why I would need to take two more sedatives after the MRI. Would there be post-traumatic stress from the MRI experience? Was this a clever intervention intended to distract me and minimize my ability to think clearly and ask anxious questions after the procedure?

Who knows? I still don't know, because no one at the hospital could explain the need for post MRI sedation and I personally felt no need for it.

Let's add confusing instructions to the list of things to avoid. Instructions should contribute to clarity rather than confusion.

3. Incomplete Instructions

While some instructions are meaningless or simply confusing, some don't go far enough.

If a patient takes a sedative, the hospital insists on having someone else drive a patient to and from the MRI. However, they say nothing about the length of time it will take before the patient can safely resume driving.

Because I asked the question, I learned that the sedatives I had taken would impair my mental capacity and motor coordination for 6-10 hours! Unfortunately, this information was not printed anywhere or volunteered by any medical personnel. If I hadn't asked the question, I would never have known the answer.

I had errands to run that afternoon. I could easily have been a danger to myself or to others on the road if I had hopped into my car once I got back from the hospital. Luckily, since I knew that I was incapacitated, my driver kindly took me on those errands.

This seems like a significant oversight in the instructions department, don't you think? If anything deserves to be crystal clear, it should be medical instructions.

However, in the hospital staff's defense, it is probably so obvious to them that sedation takes a long time to wear off, they assume that anyone would know this. As a general rule, it is best to avoid making assumptions.

Life daily provides new lessons to learn. Lessons relating to health and safety can be anxiety-ridden and difficult enough without the added stress and frustration of poor instructions. First do no harm!

Six Tips for a More Comfortable MRI Experience

The purpose of my MRI was to determine the extent of my uterine cancer prior to surgery. Regardless of the reason why you need an MRI (Magnetic Resonance Imaging) test, following these six tips will definitely make your experience more comfortable. I wish that I had known them before my own MRI.

1. Bring Your Own Music.

The MRI is incredibly loud and noisy! At various times, it sounds like a jackhammer is drilling right next to your head, a truck is backing up (next to your head), a car is having major difficulty starting, or you are going through a car wash without the benefit of a car!

Because of the noise, you are given earphones and offered a choice of music. I requested calm, lyrical classical music. The music I got was better suited to a parade than a nap. It wasn't exactly Beethoven's 9th Symphony, but it was just as energetic. This must was definitely not conducive to relaxation. When I complained, they gave me classical piano music, which was also much too vigorous.

My final choice was to have no music. However, in the future, should I ever need another MRI, I plan to bring my own CD!

2. Take a Sedative.

Regardless of the body part being scanned, your entire body will have to go into the tubular MRI. A sedative is a good idea, even if you're not claustrophobic. It is probably quite difficult to lie perfectly still for 40-60 minutes without a sedative!

3. Cover Your Eyes.

I had planned to keep my eyes closed the entire time (which I did). As an added precaution, they gave me a dry washcloth to put over my eyes- just in case I might inadvertently open my eyes. My advice- keep your eyes closed and take the washcloth as an added protection!

4. Bring Warm Socks.

The room was very cold, so I asked for two blankets. Even wearing my own socks and covered by the blankets, my feet were icy cold. So another good idea is to bring or wear some very warm socks!

5. Schedule for the Beginning of the Week.

My MRI was on a Friday before a three-day weekend, so I had to wait even longer for the radiologist to review and post the results of my test. If you have a choice, schedule your MRI for the beginning of the week. That way, you greatly increase the probability that you will get the results before the end of the week.

6. Borrow Patience!

It occurs to me is that we are called "patients" because we have to have a lot of "patience," no matter how stress inducing the wait for results may be.

The stress of having to wait probably explained my continual post-MRI craving for chocolate and carbohydrates! You may give in to the same cravings, but you'll learn what I discovered- that nothing you eat will really satisfy and fill that empty anxious place.

If you're lucky, your MRI staff will be competent, courteous and caring. My MRI staff certainly were. And if you take some of these suggestions to heart, your MRI experience should be as comfortable as possible.

How to Detect and Prevent Cervical Cancer

Thanks to widespread screening, the incidence of cervical cancer has decreased significantly. Pre-cancer lesions can be detected and removed before they become malignant. In 2010, approximately 12,200 patients are estimated to be diagnosed with cervical cancer in the United States. An estimated 4,210 will die of this disease.

Screening tests include either conventional Pap smear or liquid cytology, with or without HPV (human papilloma virus) DNA test. The current recommendations from the American College of Obstetricians and Gynecologists for normal-risk women are as follows:

1) Pap smear to start at age 21
2) < 30 years of age: every 2 years
3) > 30 years of age: every 3 years
4) Stop screening at age 65-70, after 3 normal tests during the last 10 years.

Screening is more frequent for high risk women. High risk conditions include previous cervical cancer, exposure to DES (diethylstilbestrol) as a fetus, and immunocompromised conditions (by organ transplantation drug, chemotherapy, steroids, or HIV). As with almost all forms of cancer, it is more likely to appear in people who have a family history of either ovarian cancer, or other forms of non-skin or blood cancers, for example colorectal cancer. Along with this difficulty in predicting who will suffer from the disease is the fact that its symptoms are generalized and relatively vague. Conditions that resemble gastrointestinal difficulties-indigestion, bloating, constipation, diarrhea-may be common across ovarian cancer victims. There are other symptoms, many of which could very well be mistaken for normal conditions of female physiology.

Due to the difficulty with diagnosing this disease in the earlier stages, it is often referred to as the silent killer. A term that references the fact that the disease, before it is detected, is likely to progress to the point where treatment is not very effective. It is not hopeless, however. There are some proven ways to prevent onset of ovarian cancer even though we don't know its exact cause. Having children earlier in life is one tactic, though this is exactly the opposite trend that we see amongst educated women today.

Another tactic is to have one's tubes tied. This procedure, known as a tubal ligation, dramatically reduces the chances of getting breast cancer, and it may be an especially good choice for women who have a family history of cancer. Less dramatic methods include daily consumption of birth control pills, or even aspirin (which has been shown to help, but the exact level of benefit is undetermined).

All in all we need to do more as a society to fight this silent killer. With many women waiting to have their children until their later years, it seems that it is even more important now than before to investigate the disease thoroughly.

Life Is What Happens When You Make Other Plans

A not so funny thing happened to me on my way to planning my international travels. I was diagnosed with early uterine cancer.

In the space of a few days, my life and my focus were dramatically reframed. So now, instead of getting visas and packing for Jordan and Nigeria, I'm getting medical tests and preparing for surgery.

I'm also learning a lot- about myself, about my family, and about my friends.

First of all, I've learned that my body's defensive response to devastating news is to get so sick that all that I can do is eat, read and sleep. That's how I spent the holidays. Luckily, the Christmas tree is beautiful and my cats have been happy to keep my lap warm when I sit in my rocking chair to read.

Second, as I take practical steps- to cancel my trip, to plan for work absences, to create a back up strategy if my recovery takes longer than anticipated- I realize that I am operating on automatic pilot. My emotions must be packed away with my passport. The cancer is a reality, the operation is a necessity, and everything else seems to fade in significance.

Third, I am making sure to keep breathing. That sounds funny, I know. But a new friend insists that I keep breathing and focus on healing. Oh, and eat a lot of deep green leafy vegetables. I'm doing very well on two of those three imperatives.

My family has been wonderful. Three of my brothers and one sister-in-law have all offered to leave their busy lives to be with me. My mother has asked about what she can do, since she no longer travels. My cousins have sent love and light, keeping me in their prayers. My daughter has piloted me through the maze of the University of Wisconsin Hospital, getting me to all of the various tests and appointments. My son has given me reassuring hugs.

My friends have also rallied, sending best wishes and offers of help. Even people I have only recently started to work with and get to know have offered to drive me to appointments or to sit and talk over coffee.

There is also a strong and supportive virtual community of Hystersisters who share great advice based on their personal experiences. As a result, I know what to expect, how to plan and where to go when I need answers or moral support. Since it is a worldwide organization, there are loving women awake and available at the touch of a keyboard no matter the time of day or night.

I am so very lucky. Technological advances can make this surgery as minimally invasive as possible. I have a kind and very experienced doctor and medical team. I know that I am in good hands.

At this time when I could feel most alone, instead I am nestled in the warmth of loving wisdom and concern.

I don't know what is in store for me. I don't know why this health issue is happening right now. I'm not sure what I am supposed to learn from this. Perhaps that the spirit is incredibly strong and resilient? That love and tenderness surround us? That this lifelong loner is not now, has never really been, and will never be alone.

If we are all angels learning to be human, than this is a most human experience. An MRI on Friday the 13th will reveal the extent of the cancer. Hopefully, we are catching it early before it has had time to spread. Regardless of the MRI findings, my surgery will be two weeks later on the 26th. My children and my brother will be with me- and my family and friends will be on call.

They tell me that, if all goes well, the surgery will be conducted robotically and I'll be able to go home the very next day. And if the cancer is localized, that surgery will be all that I need.

That is my hope and my prayer. Now I let go and let God.

Prevent Cervical Cancer - Visit Your Gynecologist Doctor

Pap smears are tests of a sample of cells that are taken from a female's cervix. A pap smear is used to look for a change in the cells that show cancer or conditions that may become cancer. Bear in mind that cervical cancer can be cured when it is detected early by your gynecologist doctor.

So, when should women get pap smears done? Pap screen testing should start when women reach the age of 21 and every two years after that for women who are between the ages of 21 and 29. Bear in mind that when cervical cancer is present, a common symptom is abnormal bleeding. Bleeding can start and stop between menstrual periods or it may start after douching or having sexual intercourse. Vaginal discharge may also be present. If you are experiencing these symptoms, you should consult with a doctor. Remember that pain is never an early warning sign of cervical cancer.

Myoma - What Every Woman Should Know

What is Myoma?

Myomas are non cancerous tumors that are commonly found in the uterus. It is a condition that affects almost 75% of women, especially women in their 30's and 40's.

Just last month, I was experiencing abdominal pains and headaches, so I decided to consult my doctor. The doctor did a couple of tests, CT scans, and found multiple cysts in my uterus. She Has Three Months To Live

Deep down you know what it is. Deep down, you know it's cancer, but your mind won't allow it to be spoken, somehow. The evening before, I'd received a phone call from the surgeon's secretary. Would I please report to the hospital at lunchtime the following day. Mr. Sullivan would like a word.

For some odd reason, in England, we always call our surgeons 'Mister.' I was ushered into a large, untidy office and given a chair at a large, untidy desk. Mr. Sullivan came striding in, followed closely by a male nurse. I wondered idly about the presence of the nurse, but by that time, Sullivan had seated himself opposite me and leaned his hands, fingers interlocked, on a small mountain of files he'd been carrying.

"Right, Mr. Bond. Your wife has three months."

"Oh! And then she can come home?"

"No, Mr. Bond. She has three months to live. Sorry."

Why he'd bothered to sit down in the first place, I don't know, because he simply gathered his files and cantered out of the room. The nurse hovered. I stood. "Will you be all right, Sir?" "Yes thanks, fine." I went outside into the bright sunshine and felt the tears pushing fiercely against the backs of my eyes.

Don't let go now, old son, I thought. You have to drive home. I managed the short journey and held myself in without too much trouble. I climbed out of the car, went into the house and flung myself down on the sofa. Then boy, did I let rip!

About ten minutes worth of cushion-soaking, though, and I pulled myself together. I was yet to go through the grieving cycle, of course. Far too early for that. All I'd done was to rid myself of the raw shock.

Apart from the knowledge of losing Anne, which was an all-enveloping darkness of spirit, my other great concern was that I'd turn back to the bottle. I went to the doctor, and she gave me some strong antidepressants to be taken as needed. They proved invaluable.

I had to make the usual arrangements for cremation, which was something Anne had insisted upon years before, the Death Certificate and other necessary items. My sister-in-law flew in from Canada, (Anne was Canadian), and she supported me magnificently.

I must admit now that everything's somewhat of a blur. To begin with, this happened over 30 years ago now, and memory has a way of fading the unpleasantness into a sort of oblivion. I went through all the usual grieving steps; deep sorrow, self-pity, anger. I went upstairs to sort Anne's clothes out, but of course the smell of her was upon them, and I simply couldn't continue.

The surgeon gave me his prognosis in May. One August evening I arrived home from work, just as the phone was ringing. It was the hospital, to say that Anne had died. The interesting part about all this is that it hit me like a sledgehammer. I thought I was well prepared after three months, but I wasn't.
